One of our staff members will promptly schedule you with a convenient time to see Dr. Svastits. For your convenience we also have an office map and directions available for you to print out.

Please don't delay as the majority of health conditions tend to worsen or recur without the appropriate care.

If you were involved in an automobile accident and are injured, please come in right away and bring any information regarding this accident (hospital records, car estimate, police report, etc.) with you. If you were involved in a work related injury, please come in right away and bring all information regarding your work injury claim (doctor records, attorney information, etc.) with you.

If you are taking medications, please bring ALL of your prescription medications with you for the doctor to see. If you have a PPO insurance card, please bring it with you so we can check your insurance benefits. Thank you.
